Make dental health part of your New Year's Resolution Published Jan. 6, 2012 By 1st Lt. Joe Simms JOINT BASE CHARLESTON, SC -- Col. Christopher Lentz, commander 315th Airlift Wing Aeromedical Squadron, Joint Base Charleston, S.C., encourages each Reservist to make dental health part of their New Year's resolution. The 315th Dental Services has gathered a collection of important information and websites members can utilize to receive free dental care in 2012 regardless of duty status. The Reserve Health Readiness Program is an Air Force program through which a member can receive a periodic dental exam, with xrays, for free by a preferred provider within 50 miles of their home of record. This service is available to members of any dental class status and even if they are uninsured. The form to request services is not available through the RHRP website, however. Reservists must download and e-mail the completed form to USAFRrequests@logisticshealth.com. 315th members performing a period of active duty (30 or more days of consecutive duty in support of a contingency operation) are also encouraged to contact their Tricare representative and ask about any possible Veteran Affairs benefits before separating. http://www.tricaredentalprogram.com/tdptws/info/early-tdp-enrollment.jsp The Tricare Dental Program United Concordia Dental Insurance is available to all eligible reservists. Go to www.tricaredentalprogram.com and click the National Guard/Reserve icon at the bottom of the page for the only dental plan sponsored by the DoD for Reserve sponsors and their families. The DD 2813 form that must be filled out by your dentist can be found at this site as well.
